Senior Health & Wellness Program

The Senior Health and Wellness Program is designed to provide services and activities to seniors age 60 and up who are residents of the Youngstown Metropolitan Housing Authority, International Towers, and seniors residing in Mahoning County. The program is designed to teach, develop, and empower seniors through education, exercise, and case management, which involves the seniors in the decision-making process. It assists seniors in managing multiple systems that can sometimes be complex and confusing.

Services include:

  • Home-based individual assessments
  • Referral to community-based agency to address individual family needs
  • Transportation assistance (travel training)
  • Assist with appointment scheduling
  • Medication review
  • Vision assessment
    UMCC HEAP Application

Activities include:

  • Health fair
  • Chair aerobics
  • Nutrition education
  • Finance/budget assistance

As a non-profit provider to those in need in our communities we rely on donations to help us meet the needs of those we serve. Our number one need is prayer as we seek to lift up the least of these brothers and sisters of ours. The following is a list of items that are accepted by our Centers.
